FAQs Regarding CHSM Alumni Transcript of Records

Since the closing of CHS Manila, a number of CHSM alumni have reached out to CHSAF with regard to their transcript of records and other academic documents. While CHSAF is not involved in releasing these documents, below are pertinent information that could help the alumni obtain them.

Frequently Asked Questions with regard to Transcript of Records and Other Academic Records

College and CHED-related academic records:

When can CHS Manila alumni request their transcript of records (TOR) from CHED?

  • CHED will start accepting requests from CHS Students by July 15, 2022.

How and where in CHED may the request be made?  

The alumni may contact CHED in the link below: https://ched.gov.ph/ncr/

  • For records request: please proceed to CHED-NCR Records Unit, Higher Education Development Center Bldg., C.P. Garcia Avenue, UP Campus, Diliman, Quezon City
  1. Below is the procedure: Write a letter of request by providing the required details as follows:
    • NAME of the student (Name as student; not as married name)
    • Program
    • School year when she/he graduated from CHSM or last year of stay at CHSM
    • Specify the documents needed
  2. CHED will respond and specify the date for pickup of the TOR.
  3. Fees:
  • for the Certification: P80.00 per page;
  • for certified true copy (CTC): P30.00 per page

How long is the processing time before requests can be released?

  • CHED will specify the date for pickup of the TOR

Can CHED issue a Medium of Instruction Certificate?

  • CHED will issue Certificate of English as Medium of Instruction (same rate as above).

Will CHED also issue certificates such those for honorable dismissal, or courses or subjects?

  • Honorable Dismissal will not be issued by CHED; however, they can certify that the school had been closed.

Elem and HS-related academic records: F137 FORMS

  • F137 and all related records may be claimed at the Holy Spirit School, Ilang-Ilang, Quezon City
    effective July 18, 2022

  • Requests can only be processed for those without any balance in school fees.
  • Processing Fees:
    • F137 – P500.00
    • Good Moral Certificate – P200.00

Will CHED or DepEd accept and handle the release of the UNCLAIMED documents?

*Unclaimed documents are those academic records and documents that have been prepared by CHSM for release, as received from letters of requests and emails; but were not claimed on its due dates. 

  • CHED and DepEd will not handle them.
  • The School is now closed. There are no more CHSM staff . 
  • Those who have applied for the academic records within the prescribed period ending April 30, 2022, and have not been claimed on the advised claiming date, can claim them anytime from the guard at CHSM every Monday-Friday at 8:00Am – 5:00 PM.
